    Following are the steps to make a claim for Kotak Life Term Insurance Policy:

    Step 1: Claim Intimation

    The first step is to intimate the insurance company about your claim. You can lodge your claim by visiting the claims section of the Kotak Life Insurance website. You can also intimate your claim by calling on their customer care service- 022 66057280.

    Step 2: Fill Basic Details

    You will have to fill Policy Details, Policyholder’s Details, and Claimant Details in the “Book a New Claim” Page.

    Step 3: Document Submission

    After filling in your details, the necessary documents are to be submitted via sending the Kotak Life insurance email at kli.claimsmitra@kotak.com You can submit your documents by visiting your nearest Kotak Life branch.

    Step 4: Claim Decision and Settlement

    At the final stage, your claim would be approved and settled. Payment is made to the nominee’s bank account or through cheque.

    The following documents are mandatorily required to intimate SBI Life Term insurance claim:

    • Duly-filled Death Claim Intimation Form
    • Original policy documents
    • Death certificate (Original) issued by municipality or an equal authority
    • Photograph of claimant, his present address proof, along with a photo ID proof

    Other Additional Documents:

    The other additional documents are required to intimate a death claim are:

    • A copy of claimant's bank passbook/statement with details of the active bank account
    • Medical certificate stating the cause of death
    • Medical records (consultation notes, admission notes, discharge/ death summary, investigation reports, etc)
    • Medical questionnaire
    • A certified FIR (first information report) copy 
    • Driving license copy (in case the policyholder was driving the vehicle during the accident)
