    The insurance claim procedure is convenient and user-friendly. The claimant or the nominee should inform the company about the policyholder’s death. On receiving the information, the claim settlement officer will forward a list of documents along with the respective application forms to the nominee. The nominee should complete the forms and submit all the documents listed out, as necessary. The claim form and list of papers is also available on the official website. On receipt of all the documents, the claim is registered. The claims settlement department can call for additional documents if required for processing the claim. After verifying the documents, the company will provide the decision on the claim.

    C. Additional Documents, In the Case of Hospital Death.

    • Physician’s Statement – Form P
    • Treating Hospital Certificate – Form H
    • Employer (Form E) Or School/College certificate (Form S)
    • Hospital treatment records

    D. Documents Necessary in the Case of Accidental/Unnatural Death

    • Copy of Police FIR, Panchanama, and Police Complaint
    • Copy of post-mortem report, autopsy, and viscera report
    • Copy of Final Police Investigation report/charge sheet
